Delete My Wellness App Account

At Drs. Else & Robertson Chiropractors | Wellness, you may request deletion of your
Wellness App account and associated app data at any time.
To request deletion, please email us at hello@familychiropractic.co.za with the subject line: Delete My Wellness App Account

Please Include:

  • Your full name
  • The email address used to register for the App
  •  Your Mobile Number
  • Your request to delete you Wellness App account
What will be deleted
 
When your request is approved, we will delete or deactivate your Wellness App account and remove personal information stored specifically for use within the app, including your app profile and app-access information.
 
What may be retained
 
Clinical records, treatment notes, appointment history, invoices, payment records and other healthcare-related records may need to be retained by the practice where required by applicable law, professional rules, medical-aid requirements, accounting requirements or record-retention obligations. These records are managed separately from your Wellness App account.
 
Timeframe
 
We aim to process verified account-deletion requests within 30 days. Some information may be retained for longer where required by law or for legitimate administrative, security or financial-record purposes.
